dc.description.abstractLa hipofosfatemia ligada al cromosoma X (XLH) (OMIM 307800) es la forma más común de raquitismo hereditario. Se produce por mutaciones del gen PHEX, se caracteriza bioquímicamente por hipofosfatemia con hiperfosfaturia y concentraciones séricos de 1,25vitD bajas y de FGF23 y altas. Clínicamente, los pacientes presentan raquitismo, osteomalacia y deformidades óseas. Utilizando el modelo animal del XLH, el ratón Hyp, hemos intentamos dilucidar cómo los tratamientos convencionales (suplemento de fósforo, GH), y / o los nuevos fármacos en desarrollo (inhibidores de la vía FGF23) pueden afectar el desarrollo óseo y a la placa de crecimiento (órgano responsable del crecimiento en longitud). Los estudios llevados a cabo a lo largo de esta tesis doctoral nos han permitido concluir que: 1) los ratones Hyp tenían una desregulación en las proteínas involucradas en el proceso de hipertrofia de condrocitos, así como en la proliferación y apoptosis celular. 2) Parece que la mutación afecta sobre todo al patrón columnar y la organización de la zona hipertrófica de la placa de crecimiento, así como a la mineralización de la unión osteo-condral y por ende a la estructura trabecular. 3) Los tratamientos tradicionales, aunque tienen un efecto positivo sobre la calidad del huesos y la longitud final, no fueron capaces de normalizar la estructura de la placa de crecimiento, incrementando así el riesgo de sufrir deformidades óseas en pacientes pediátricos con XLH. 4) Aunque bloquear la acción de FGF23 resultó ser un tratamiento altamente efectivo tendiendo a normalizar la morfología de la placa de crecimiento, el efecto positivo sobre el hueso cortical fue menor en comparación con los grupos tratados con GH. 5) Finalmente la combinación con anti-pERK1/2 más GH fue el tratamiento más beneficioso, debido al efecto sinérgico positivo de ambos fármacos sobre la estructura de la placa de cremiento, así como en la densidad mineral ósea. Finalmente, concluimos que, aunque los tratamiento con Pi y GH mejoran el raquitismo y la osteomalacia, su falla en la normalización de la estructura de la placa de cremiento, no reduciría la probabilidad de sufirir deformidades óseas, Sin embargo, este estudio experimental parece indicar que el tratamiento con GH (solo o combinado con Pi) puede ser positivo en pacientes con deterioro evidente del crecimiento, sin embargo, un seguimiento cercano debe ser realizado debido a que existe un riesgo potencial de exagerar la desproporción corporal. En este aspecto las terapias relacionadas con FGF23 surgen como un tratamiento más eficaz para los niños con XLH, siendo la combinación con inhibidor de la vía MAPKi más hormona de crecimiento el de mayor alcance, surgiendo, así como prometedora terapia para pacientes pediátricos con raquitismo hipofosfatémico. EN INGLÉS: X-linked hypophosphatemia (XLH) (OMIM 307800) is the most common form of hereditary rickets and is caused by mutations of the PHEX gene. XLH is characterized biochemically by hypophosphatemia with hyperphophaturia, low serum concentrations of 1.25 Vitamin D and high FGF23 serum levels. Clinically, patients have rickets / osteomalacia / bone deformities. The main aim of this doctoral thesis study was trying to elucidate hte molecular mechanism underlying growth impairment in XLH pediatric patients. Using the animal model of the disease, the Hyp mouse, we investigated how conventional treatments with phosphorus, Gh and/or recently developed drugs (FGF23-pathway inhibitors) affect bone and growth plate development. We found that: 1) Hyp mice had a dysregulation in the proteins involved in the process of growth plate's chondrocyte hypertrophy, as well as in proliferation and cell apoptosis. 2) XLH disturbed the columnar pattern and organization of the hypertrophic zone of the growth plate and the mineralization of the osteochondral junction and thereby the trabecular structure. 3) Traditional treatments, although exerting a postive effect over ht ebone quality and the final lenght, were not able to imprve the aberrant growth palte structure, thereby potentially increasing the risk of suffering bone deformities of XLH pediatric apatients. 4) Although blocking FGF23 action was highly effective and tended to normalize the growth plate morphology, which might reduce the risk of sufffering bone deformities, the postivive effect over cortical bone density was lower compared to GH treated Hyp mice. 5) Lastly, a combined treatment with anti PERK1/2 plus GH was the most beneficial treatment, due to the positve synergistic effect on both the growth plate structure and the bone mineral density.spa
